Intercepts and modifies fetch Duolingo's API responses to give Duolingo Super.
Yes, this is stated on the GitHub which is linked inside the script description. it is impossible to grant a server sided subscription. If you want to use the script on different devices there are methods to do so on the FAQ section: https://github.com/apersongithub/Duolingo-Unlimited-Hearts

it is actually possible to grant a server-side subscription through exploiting a bug (duolingo PRO can do it) but it's only a 3 day preview, you can claim it every 3 days to make it basically an unlimited subscription
You are right, I did forget about this bug. Unfortunately, the super trial bug that you are talking about does not work consistently across all accounts and eventually stops working after redeeming it a couple of times. People often complain about its lack of consistency in their discord. Also the duolingo pro userscript is personally not recommended by me to use as their script is not open source, they send data to their own servers (which is not even required for the script to work they do it to get you to pay money), and have access to your duolingo cookie and other account data (such as email, location, etc). The userscripts and extensions that I have made are all processed locally with no external servers being accessed. Use what you please but remember to be cautious!
Duolingo Pro's discord comment below:
it works, but the changes are only client-side, so if you disable the script or log in on a different device, the super duolingo will not be there
