You're absolutely right — the gear should ideally stay out of the way when fullscreen is active or when viewing videos directly.
🔧 I’ll be adding an auto-hide gear in fullscreen feature in the next update.
Also considering an optional toggle to completely hide the gear on video pages, for those who just want the features without the button.

I noticed some scripts you can change the options in the TamperMonkey dropdown menu, if it's not too much of a hassle maybe it's better having than not.
My knowledge in programming is very basic, but I think it might be easier to add the gear element straight into the page as opposed to a floating button, like beside the YouTube (or Premium) logo, or on the top right.
Either way, thanks for your work, and I'm looking forward the next update!
Need an option to hide the "Gear" when video is full screen, or maybe the gear shouldn't even appear at all when you're in a video page.
That aside, it works very well, and I find it very useful.
