Uncheck [ ] disable DASH playback in settings of the script (your screenshot#1) because youtube can only show 1080p and higher using DASH. BTW I think it might be a good idea for the author of the script to add a small notice about this in the label of the checkbox.

Yea, 1080p is only available with DASH enabled. It's something weird Google did.
Everytime quality set maximum to 720p, even if 1080p is available (Firefox 33.0.2)

2) open a video with fullhd 1080p (script is OFF) http://picsee.net/upload/2014-10-30/982294ad6170.jpg
3) enable script, refresh video http://picsee.net/upload/2014-10-30/94fc9ba125b4.jpg
why it not 1080 p ? Can you fix it?
