I just replicated your issue and it's thankfully not the script itself but Opera / Tampermonkey.
To get it running you need to follow this FAQ https://www.tampermonkey.net/faq.php#Q209 to enable developer mode. Then you need to restart your browser and it should work, at least then it did for me on a new Opera install.
The script has been down for a while, six months to a year. I tried reinstalling Tampermonkey, the script itself, it doesn't help. I didn't want to get into the code because I understand very little about it.
The main reason why I used this script is that as the video quality decreases, so does the sound quality, which annoyed me a lot.
Re-posting this issue cause you've asked me what script version I'm using and you didn't answer afterwards.
Script version - 1.2.7
