Table of Contents Everywhere

On pages which do not have a Table of Contents, but should do, create one! (I actually use this as a bookmarklet, so I can load it onto the current page only when I want it.)

Feature request:

- keyboard shortcut to toggle visibility of TOC panel
- keyboard shortcut to toggle collapse
- a setting to control default visibility: TOC panel is hidden everywhere until a user press shortcut to show it (bool flag in the code is enough, no need fancy UI for that)
