Icons for settings, info, and reload missing

Tested in both firefox and waterfox 48.0.2
No problem in chrome

Seems to work fine for me in firefox, have an example URL where it isn't working?

This happens on all websites.

I tried a few things and it seems that blocking the maxcdn.bootstrapcdn.com domain is the cause. I reproduced it in chrome aswell.

I'm assuming the icons are loaded from that domain and that is the problem i was experiencing.

Unblocking the maxcdn.bootstrapcdn.com domain fixed the problem.
