Make login/password boxes on selected sites compatible with Bitwarden auto-fill

The problem is that on some sites Bitwarden addon cannot recognize login or password input boxes. This script makes selected sites compatible with Bitwarden auto-fill by adding recognizable id attribute to the right input elements.
This script can be potentially helpful for other password keepers.
The list of supported sites:
1. kyivstar.ua: phone number as a login and PUK2 as an alternative confirmation (if you experience troubles with getting sms). In Bitwarden you should keep full phone number as a username and PUK2 as a custom field named "PUK2".
2. privat24.ua: phone number as a login. In Bitwarden you should keep the phone number without "+" and country code.
3. lifecell.ua: phone number as a login. In Bitwarden you should keep the phone number without "+" and country code, but with leading 0.
4. novaposhta.ua. In Bitwarden you should keep full phone number as a custom field named "phone".
5. otpsmart.com.ua.
6. fora.ua: phone number as a login. In Bitwarden you should keep the phone number with country code but without "+".
